For those of you coming to pilates for the first time: please bring a completed New Client Information Form, below, to your first session. If you are unable to download and print the PDF below, please come to your appointment 15 minutes early. There are blank forms available on my desk. This will allow us to use your time more effectively. |

Payment is due on or before the day of class unless previous arrangements have been made. Twenty-four hours notice is required for the cancellation of an individual session. In cases where 24 hours notice is not given, the session will be charged in full.
Although packages are generally considered non-transferrable (e.g., they may not be purchased with the intent of sharing with another individual), we do allow students to gift sessions to friends or family when missing an appointment scheduled within 24 hours. |
| Advance payment is required to reserve your spot in class. Once payment has been received, notice of five days prior to the first day of a series class is required to cancel your reservation. If notice and payment have not been received but you have registered for a class, you are responsible for payment for the class in full once it has begun, although class attendance is transferrable if you are no longer able to attend. |
